This form is confidential and will become the property of DHBI and will not be returned to the student. Should the applicant be denied or otherwise not enroll, DHBI is under no obligation to disclose the contents of the application to the applicant, nor will DHBI release any information to a party legally unrelated to DHBI or DHBC unless required by law to do so. Upon enrollment, the application becomes a part of the student’s permanent academic record.
